The PSU & OHSH IHI Open School Chapter is hosting an interdisciplinary symposium and networking event focused on Oregon’s implementation of the Patient-Centered Primary Care Home (PCPCH) Program.

This event will highlight Oregon’s model, implementation strategies, evidence of effectiveness, and lessons learned to improve health and health care for Oregonians while lowering costs.

Expert speakers are:

Sherril Gelmon, DrPH (Professor of Public Health, Portland State University);

Nichole Merrithew, MPH  (Director, PCPCH Program, Oregon Health Authority); and

Kate Elliot, MA  (Oregon Healthcare Quality Corporation, representing the Patient Centered Primary Care Institute)
